Applicants are required to mail an application form and related documents to the following address before the deadline.

Application Materials should include:

Applicants are required to mail an application form and related documents before the deadline.

1. Application form download Application form

2. Health Certificate with HIV Test Result (within the past 6 months).

3. A copy of passport photo page. Your passport must be valid for at least 6 months.

4. 2 recent passport photos. You may submit your passport photo upon arrival.

5. A copy of the highest level diploma.

An original bank statement. Dated within the last 3 months showing at least US$ 2,500 from the applicant's or his/her sponsor's account. The bank statement must be written in English on official bank letterhead, contain an account number, the amount of money available in U.S. or foreign currency and MUST be signed and dated by a bank official no more than three months prior to the date you are applying on. The sponsor(s) name(s) must also be mentioned on the Bank Statement. A valid proof of the relationship between the applicant and sponsor should be provided. Ex. birth certificate.
